These are standard Wisconsin State Public Defender private bar forms in Microsoft Word format. Hope you find them useful. They are copyrighted but feel free to redistribute them in unmodified form with all documentation. These are not "approved" or sanctioned in any way by the Office of State Public Defender.

They are available in zip and Word template (dot) files. The documentation file for the payment voucher is in the zip file and is highly recommended. These are "protected" Word forms which means that they are fill-in-the blank. You use your tab key (not the Enter key) or mouse to move from field to field. The payment voucher does most of the calculations. There is no charge for these forms. Feel free to pass them on without modification to others.

P.S. If anyone knows how to get TimeSlips (version 9) to print a bill that separates into columns for court/out-of-court/travel I would sure like to find out how you did it.
